*1 There is no food safety manager certified by this Department (CFSM) on duty as required. A CFSM must be on duty at all times the facility is open and/or food is being prepared. A completed application(SA-71)for CFSM, or proof of enrollment in an approved safety class must be submitted to the Department.  New Violation.
*11 *A spray bottle filled with water is stored directly in the ice machine. The PIC removed.  New Violation. To be Corrected By: 02/17/2023
*14 *Upon arrival, the facility had no sanitizer for the three compartment sink. Employee went to another market to provide tablets and test strips. Always keep sanitizer available.  Corrected On-Site.  New Violation.
*14 *Knives stored in the designated clean knife rack are dirty and must be cleaned to sight and touch.
The ice machine is accumulating a mold-like substance and must be fully cleaned and sanitized. (Dispose of ice).

35 Juices in the self service refrigerator do not bear labels. They must have the following:

(1) The common name of the food.
(2) If made from two or more ingredients, a list of ingredients in descending order of predominance by weight, including a declaration of artificial color or flavor and chemical preservatives, if contained in the food.
(3) An accurate declaration of the quantity of contents.
